Invisible Condition

This voluntary program allows a symbol, “sticker” to be placed on your driver license or identification card upon request.  The invisible condition is used to notify a law enforcement officer or other first responders of a physical or mental health condition that may interfere with the ability to communicate with others.

For example:
  • Communication impediment
  • Hearing loss
  • Autism spectrum disorder
  • Drug allergy
  • Alzheimer’s disease or dementia
  • Post-traumatic brain injury
  • Schizophrenia
  • Epilepsy
  • Developmental disability
  • Down syndrome
  • Diabetes
  • Heart condition
  • Other conditions identified by a health care professional

To qualify, you must submit the completed Invisible Condition Request form, signed by your health care professional, to the Driver License Division by mail, fax, email, or office location.  The division will add the information to your record and provide you with a symbol.

The symbol may be removed from your record by completing the Invisible Condition Removal form.
